This year's golf outing will be held at Green Brook Country Club, North Caldwell, New Jersey on September 20, 2004 beginning with a shotgun start at 1:00 P.M.

Below is the registration form for your participation in the tournament as a player, sponsor, promotional gift donor or dinner attendee. For those participating in the golf tournament, you will receive the following.

• Use of practice range and putting green (weather permitting)
• Grilled Lunch on the Terrace and in the Green Bee
