This year's golf outing will be held at Green Brook Country Club, North Caldwell, New Jersey on September 20, 2004 beginning with a shotgun start at 1:00 P.M.
Below is the registration form for your participation in the tournament as a player, sponsor, promotional gift donor or dinner attendee. For those participating in the golf tournament, you will receive the following. We hope to see you on September 20th for a fun-filled day of festivities. A threesome at Baltusrol Golf Club, one of the country's most prestigious clubs and home to 7 U. S. Opens and the 2005 PGA. Must be used in 2004.
Once again outing participants and guests will have an opportunity to bid on a Lasik Surgery Procedure performed by Dr. Barrie Soloway, a prominent New York City ophthalmologist who has treated movie stars and professional sports stars. Last year's high bid went to Dr. Anthony Laneve.
Dr. John Campolattaro of Fairfield, NJ has donated a Zoom Dental Whitening Procedure. This procedure is the same one seen on the television show Extreme Makeover .
